Type 2 Diabetes Remission Through Structured Diet Programs
A large UK trial confirmed that a low-calorie diet intervention can send Type 2 diabetes into remission for up to five years, reducing dependency on medication.
The DiRECT Trial Findings
The Diabetes Remission Clinical Trial (DiRECT) followed 300 participants through a low-calorie total diet replacement program (800 kcal/day for 12-20 weeks). At five years, nearly a quarter of participants maintained remission, defined as HbA1c below 6.5% without diabetes medication.
The Role of Weight Loss
Remission is strongly correlated with the amount of weight lost and sustained. Losing 15kg or more was associated with remission in the majority of participants. The mechanism involves reduction of liver and pancreatic fat, restoring beta-cell insulin production.
Community-Based Implementation
The NHS has rolled out a national Type 2 diabetes remission program based on DiRECT findings. Community health workers and culturally tailored dietary counseling are key to making this intervention accessible to diverse populations.
